  • by submitting this form I am changing the contact details associated with the rented premises referred to in section 1 of this form.
  • I understand that giving false or misleading information, including giving information that is false or misleading because something has been omitted, is a serious criminal offence.
  • the information provided in this form is true and correct and I agree to notify the ACT Revenue office within 30 days and provide full details of a change in circumstance including change of contact or address details and property status.
